I am unable to purchase an out of plan add on using the SMARTY app on iOS 17.3. I check the box accepting the terms but clicking on “Pay with Apple Pay” or “Choose payment method” does nothing.

You cannot port forward on a SMARTY connection. SMARTY use CGNAT!

iD Mobile will be offering eSIMs to their customers in the very near future. If all goes to plan, the end of the month. So I don't think it will be long before SMARTY offers them too!

SMARTY don't haggle. If you found a cheaper plan on another network then take it.

So switch away. It's not like SMARTY are holding you hostage for two years! Every network will have area's where they have little to no coverage. Local councils don't allow all four networks to install masts at the same locations. It is what it is.
