This is the advice I found with regards to finding your SMARTY PUK code.....

You can get a PUK code for a SMARTY SIM card by contacting SMARTY via web chat or checking the packaging that came with the SIM card. 

How to get a PUK code from SMARTY 

1. Contact SMARTY via web chat (scroll to the bottom of the page)

2. One of their agents will provide you with the PUK code

How to get a PUK code from the packaging 

1. Check the packaging that came with the SIM card
2. The PUK code is usually on the back of the packaging

What happens if I enter an incorrect PUK code?

You have 10 chances to enter a correct PUK code. After that, your SMARTY SIM will be blocked. 

How to unblock your device 

• Enter your PUK code

• Choose a new 4-digit PIN

• You'll be unblocked
