My insurance is not accepting this as formal proof and highly doubt any other would either. 

That was a non-answer.  No insurance company would find that an acceptable form of proof, no address,  no IMEI number, you can't even export it as PDF.  This whole experience has made me realise that this so-called cheap SIM only deal is not worth it...

What was the result of this? Its a fairly standard insurance requirement.
