Where do I find my username
and password needed to use the SIM card in a dashcam

ReubenS_2808875 Yopui may find it difficult to use a sim card in a dash cam unless it alwayshas an adequate signal wherever you are.
Are you certain that the dash cam does not need a micro SD card to simply store the pictures? For many of these cameras, you then form a network between the camera and your phone to download any film/pictures that you need.
ReubenS_2808875 I will assume this is about the APN settings. Short answer: the username and password fields should be left blank.
